Primary gastrointestinal system (PGIS) lymphomas are rare. It is very difficult to diagnose PGIS lymphoma preoperatively.
A 3-year-old male patient with an intraabdominal mass was presented in this study. Lymphoma was considered as the preliminary diagnosis. Fine-needle biopsy was performed, and pathological examination of the biopsy specimen showed CD20 (+) aggressive B-cell lymphoma. Intestinal perforation developed on the third day of chemotherapy, and emergency laparotomy was performed. Colon resection from the terminal ileum to the splenic flexura and ileostomy were performed. Histopathological examination revealed high-grade B-cell lymphoproliferative disease. The patient died on the 6th postoperative day.
Although rare, PGIS lymphoma should be kept in mind when diagnosing patients who present with GIS symptoms. Keywords: Intestinal lymphoma, perforation, surgery, child

Primer gastrointestinal sistem (PGİS) lenfomaları nadir görülür. PGİS lenfoma tanısının ameliyat öncesi konulması oldukça zordur.
Bu çalışmada, intraabdominal kitle tespit edilen 3 yaşındaki erkek olgu sunuldu. Ön tanıda lenfoma düşünüldü. Yapılan ince iğne biyopsisinin patolojik incelemesi CD20 (+) agresif B hücreli lenfomaydı. Kemoterapinin 3. gününde intestinal perforasyon gelişen hastaya acil laparotomi yapıldı. Terminal ileumdan splenik fleksuraya kadar kolon rezeksiyonu ve ileostomi açılması işlemleri yapıldı. Histopatolojik incelemede B hücreli yüksek grade’li lenfoproliferatif hastalık olarak tanımlandı. Hasta postop 6. gün exitus oldu.
Sonuç olarak; GİS semptomları ile başvuran olgularda nadir de olsa PGİS lenfoma akılda tutulmalıdır. Anahtar Kelimeler: İntestinal lenfoma, perforasyon, cerrahi, çocuk
